Based on a review of facility documents, and interview with facility staff (EMP), it was determined that Tyrone Hospital failed to adopt an advisory group with at least one MD or DO, and one or more physician assistants, nurse practitioners, or clinical nurse specialist to develop and review patient care policies at least annually.

Findings include:

Review of Tyrone Hospital Medical Staff Bylaws, reviewed/revised April 2015, revealed no documented evidence of an advisory group or committee with at least one MD or DO and one or more physician assistants, nurse practitioners, or clinical nurse specialist to develop and review patient care policies at least annually.

1) An interview was conducted with EMP2 on May 19, 2015, and confirmed the above findings. "We do not have a committee or a group that develops patient care polices."
