Based on observation and interviews, the facility failed to properly maintain exit egress as per NFPA 19 2.2.2.6. This standard deficiency practice affected one (1) of seven (7) exits in the facility on the day of survey.
Findings include:
On December 11, 2017 at 11:40 AM, observation revealed locked magnetic door from the Purchasing Area to the exit egress. The locked, magnetic required a special code to open did not release upon activation of the fire alarm system.

Based on observations, the facility failed to properly protect corridors as per NFPA 101 section 19.3.6.1 exception 7. This standard deficiency practice affected one (1) of two (2) smoke compartments in the facility on the day of survey.
Findings include:
On December 11, 2017 at 10:50 AM, observation revealed the Nurse Locker Room was not protected by automatic smoke detection system. The Nurse Locker Room was open to the main corridor of the facility and was unable to resist the passage throughout the facility. The facility was not protected by automatic sprinkler system.

Based on observations and interviews, the facility failed to properly maintain smoke barrier walls for the purpose of providing 1 half hour fire resistance as required by NFPA 101 section 19.3.7.3. These standard deficiencies practice affected two (2) of two (2) smoke compartments in the facility on the day of survey.
Findings include:
On December 11, 2017 at 11:50 AM, observation revealed unsealed penetrations in the smoke barrier walls near the following areas of the facility. The unsealed penetrations in the smoke barrier walls were located above the ceiling in the Physicians Work Room and above the ceiling in the Outpatient Waiting Room of the facility.

Based on observations and testing, the facility failed to properly maintain door openings in smoke barrier walls as per NFPA 101 section 19.3.7.8. These standard deficiencies practice affected two (2) of two (2) smoke compartments in the facility on the day of survey.
Findings Include:
On December 11, 2017 at 12:00 PM, observation revealed the magnets on all the smoke doors re-energized and locks upon silencing the fire alarm system. This enable the smoke barrier doors to be held open upon activation of the fire alarm system. The smoke barrier doors were located near the Nurse Station, near the Front Entrance and near the Conference Room of the facility.
