Observation of the dietary area at 10:00 AM on 02/15/2018 along with hospital employee #55 revealed a collection of an accumulation visible dust attached and hanging down from the interior of the vent and duct area located in the dietary receiving area. Additional observation included 2 floor drains that were approximately 3 feet by 6 inches located in front of the walk-in cooler and freezer units that had visible dirt and debris coating the bottom area of the drains. Visible debris and dust was also observed throughout the dietary floor area near the baseboards, corners of the flooring and beneath dietary equipment. Tile grout lines also appeared to be dirty and dark colored. These finding created an environment for pest and bacterial growth.

Interview with employee #55 during onsite observation of the dietary area revealed when asked if the condition of the floor and the debris and dust in vent was acceptable, employee #55 confirmed it was not acceptable and something needed to be done about it.

Interview with the Chief Nursing Officer employee ID #56 at 11:45 AM on 02/15/2018 when asked about the scheduling of cleaning for the kitchen/dietary area stated, "it is random for cleaning the kitchen area."

Review of the hospital's Policy Number DS 1300.08 Effective date: 10/21/2014; Dietary Services reads in part," Purpose: To establish guidelines to maintain infection control ......Dietary manager will safeguard all food by ensuring proper storage, maintain a sanitized environment ..."

Review of invoices provided for services from contracted company A documented random cleaning of kitchen area. Invoice dated 02/07/2017 for exhaust system cleaning, invoice dated 03/12/2017 showed kitchen exhaust system cleaning and floor pressure washing, invoice dated 05/19/2016 showed kitchen exhaust system cleaning and floor cleaning and invoice dated 08/10/2017 showed kitchen exhaust system cleaning.
