Based on observation and interview, the facility failed to maintain and test fire extinguishers as required. The deficient practice could affect all patients and staff in the facility. This facility had a census of 0 patients. Findings include:
Observation of the fire extinguishers on 5/5/2011 between 2:00 p.m. and 3:15 p.m., there was no date or initials marked on the extinguisher tags as required for the month of March 2011 in the entire facility. Observations were acknowledged by the by Maintenance Staff A at the time of the observations.

Observation on 5/5/2011 between 2:00 p.m. and 3:15 p.m., showed that the three power taps in the ER did not meet the proper UL listing for use in patient care areas. Observations were acknowledged by the by Maintenance Staff A at the time of the observations.
