Based on record review and staff interview the facility failed to assure that appropriate construction standards are met as required by the life safety code for a non-sprinklered building. This deficient practice would not prevent the spread of fire and smoke, affecting 2 of 4 smoke compartments. This facility has a capacity of 14 with a census of 3 at the time of survey.
FINDINGS INCLUDE:
During the tour on 1/4/10 it was observed in the record review and staff interview that during the Routine Inspection dated April 1, 2009 by the Kansas State Fire Marshals office, the basement area has a drop in tile ceiling that is not rated for 1 hour and the steel supports that are not protected. As of this date this deficiency has not been corrected.
The Administrator was aware of this finding and stated that the facility has previously asked for a Waiver request asking for additional time to complete.

Based on observation and staff interview the facility failed to assure that 1/2" plywood meets interior finish requirements, which may cause the spread of smoke and fire. The facility has a capacity of 14 beds with a census of 3 residents. This will affect 1 out of 4 smoke compartments.
FINDINGS INCLUDE:
During the tour on 1/4/10, 10:20 A.M. ,it was observed that the interior finish for the corridor ceiling was 1/2" plywood installed by the Murray Construction along the main corridor by the Nurses station, approximately 50' East and South, for a temporary time during the construction project.
The Maintenance Supervisor and Construction Manager were present and aware of the finding.

Based on observation and staff interview the facility failed to assure that battery powered emergency lighting had a duration of at least 1 1/2 hours, failing to ensure that emergency lighting would operate during a power outage. This facility has a capacity of 14 beds with a census of 3 residence during the time of the survey. This affects 1 of the 4 smoke zones.
FINDINGS INCLUDE:
During the tour on 1/4/10 at 11:50 A.M. is was observed that the battery emergency light in the records room did not operate when tested.
The Maintenance Supervisor was present and aware of the findings.

Based on observation and staff interview the facility failed to assure that exit signs were properly located in order to direct occupants through a proper path of egress to a safe area, not the hazardous laundry room. This facility has a capacity of 14 beds with a census of 3 residents at the time of the survey. This will affect 1 out of 4 smoke zones.
FINDINGS INCLUDE:
During the tour on 1/4/10 at 11:05 A.M. it was observed that an "EXIT" sign is located at the laundry room door directing the exit pathway through the laundry room, that has an exit door with an "EXIT" sign.
The Maintenance Supervisor and Administrator were present and aware of the findings.

Based on observation and staff interview it was observed that surgical gloves were placed over two smoke detectors in the corridor by the nurses station. This deficient practice will delay the activation of the fire alarm system. This facility has a capacity of 14 beds with a census of 3 residents during the time of the survey. This affects 4 out of 4 smoke zones.
FINDINGS INCLUDE:
During the tour on 1/4/10 at 10:40 A.M. it was observed that surgical gloves were covering smoke detectors in the corridor South of the Nurses station where construction is underway.
The Maintenance Supervisor and Contractor were present and aware of the findings.

Based on observation and staff interview the facility failed to assure that the corridors are free of obstructions, which will affect exiting the facility in a timely manner. The facility has a capacity of 14 beds with a census of 3 residents at the time of the survey. This will affect 3 out of 4 smoke compartments.
FINDINGS INCLUDE:
During the tour on 1/4/10 at 10:25 to 11:50 A.M. it was observed that the corridors on the main floor were obstructed by medical and food carts, construction equipment, buckets, vacuum, new sink in a box, two buckets, two ladders, and portable scaffolding. The South basement exit corridor has a dozen plants, storage cabinet, table and chair.
The Maintenance Supervisor was present and aware of the findings.

Based on observation and staff interview the facility failed to assure that residents are not residing in a construction zone area which may jeopardize the safety of the residents in an emergency situation.
This facility has a capacity of 14 beds and has a census of 3 residents at the time of the survey. This affects 1 out of 4 smoke zones.
FINDINGS INCLUDE:
During the tour on 1/4/10 at 12:20 P.M. it was observed that 3 residents are residing in a construction zone area where corridor ceiling tiles have been removed and surgical gloves have been placed over the smoke detectors due to dust caused by construction.
The Maintenance Supervisor was present and aware of the findings.

Based on observation and staff interview the facility failed to assure that the electrical wiring and equipment is in accordance with NFPA 70, National Electrical Code. Use of extension cords may cause a fire or smoke hazard and should not be used in lieu of permanent wiring. This facility has a capacity of 14 beds with a census of 3 residents at the time of this survey. This will affect 2 out of 4 smoke compartments.
FINDINGS INCLUDE:
During the tour on 1/4/10 the following was observed:
1.) At 11:30 A.M. a portable heater was powered with an extension cord located in the basement conference room office. The extension cord was removed at the time of survey and heater was plugged directly into a wall outlet.
2.) At 11:32 A.M. an extension cord was piggy backed with a power strip for the computer and printer located in the basement conference room office.
3.) At 12:20 P.M. four pieces of medical equipment were plugged into a power strip in the storage room across from main floor nurses station.
4.) At 10:40 A.M. the kitchen wall mounted fan is plugged into an extension cord.
The Maintenance Supervisor was present and aware of the findings.
