Based on observation and interview the facility did not provide fire safety to patients and staff.

Findings include:

Observation during the tour on 7/15/11 at 10:30 am revealed the door to Central Supply (Purchasing) was open to the corridor. This regulation requires this exit corridor door to be maintained in the closed position.

Based on document review and interview the facility did not provide testing of all Components of the Life Safety System.

Findings include:

Review of the facility ' s equipment maintenance documents on 7/14/11 revealed no record of Emergency Light Testing. Interview with Staff # 3 on 7/14/11 revealed logs are not maintained for Emergency Light Testing. Battery Operated Emergency Lights are located in the OR ' s, and the hospital ' s extension sites. Emergency Light Testing requires a 30 second monthly and 90 minute annual test.

This is a repeat deficiency.

--Observations during the tour on 7/13/11 between 9:45 am and 12:30 pm revealed fire extinguisher tags were not marked for monthly inspection as required by this regulation. Interview with Staff # 3 on 7/13/11 at 10:30 revealed logs and inspection reports are not maintained by the facility.

--Observation during the tour of the Chautauqua Extension Clinic on 7/14/11 at 9:00 am revealed the fire extinguisher inspection expired in 2009.
