Based on observation at the time of inspection, the facility failed to provide a protected outdoor med gas cylinder storage. Cylinders were not protected from inclement weather.
Storage of Cylinders and Containers. Cylinders stored in the open shall be protected as follows: (1) Against extremes of weather and from the ground beneath to prevent rusting, (2) During winter, against the accumulation of ice or snow, and (3) In summer, screened against continuous exposure to direct rays of the sun in those localities where extreme temperatures prevail." - NFPA 99, 2002: 5.1.13.5.

Based on observation at the time of inspection, the facility failed to provide properly updated directories to the essential electrical panels.
Panel directories shall be labeled to legibly identify the purpose or use of each circuit - NFPA 70, 2002: 408.4.
Based on observation at the time of inspection, the facility failed to provide proper labeling to the existing disconnect. All disconnects associated with the essential system must be properly labeled.
NFPA 70, 2011, 110.22 Identification of Disconnecting Means. (A) General. Each disconnecting means shall be legibly marked to indicate its purpose unless located and arranged so the purpose is evident. The marking shall be of sufficient durability to withstand the environment involved.
Based on observation at the time of inspection, the facility failed to provide properly secured cover plates for junction boxes. At the time of inspections, various locations such as the fire riser room and the mechanical room had open J boxes.
All junction boxes of any nature are required to have properly secured cover plates with all wiring secured inside. - NFPA 70, 2002: 314-28(C) and 314-24.
Based on observation at the time of inspection, the facility failed to provide properly identifiable essential panels.
All boxes and enclosures (including transfer switches, generators, and power panels) for emergency circuits shall be permanently marked so they will be readily identified as a component of an emergency circuit or system." - NFPA 70, 2011, 700.10. They shall be labeled "LIFE SAFETY", "CRITICAL", or "EQUIPMENT" as applicable.
