Based on review of the medical record and staff interview, it was determined that the facility failed to ensure that baths were consistently provided for 1 of 3 patients (#1) whose records were reviewed.
A review of the medical record revealed that patient #1 was hospitalized for four (4) days. During the hospitalization, on-going nursing assessments revealed documentation that indicated the patient required assistance with bathing. The progress note documentation revealed that the patient received a bath on the third day of hospitalization. The record lacked documentation related to baths on other days of hospitalization.
An interview at 2:20 p.m. on 02/24/ 2010 with the Assistant Nursing Director of the unit where the patient was admitted confirmed that there was no documented evidence of baths having occurred other than on the third day of hospitalization. The interviewee related that the patient care technicians were assigned baths and had the responsibility for doing the task and for documenting that the bath had been completed. The director was unable to provide any information or explanation as to why there had not been follow up by the nurse who was responsible for the patient's care.
