Based on the facility walk-through life safety code inspection on 02/26-27/2014 wirth the VP administration, safety manager, and director engineering, it was noted that there was not signage/notations on teh faces of the Fire Alarm Control Panel or fire alarm booster panels, to indicate the electrical panel and electrical circuit numbers supplying power to the FACP and booster panels as per NFPA 72, 4.4.1.4.2..3.

Based on the onsite walk-through inspection on 02/26and 27/2014, with the VP, Director of Engineering, Safety Manager, and maintenence personnel, it was observed that there was not identification markings located on the face of the Fire Alarm Master Control Panel or other booster Panels of the Fire Alarm system, to identify the electrical panel or the power circuit within the panel(s) providing power to the Fire Alarm System as required by NFPA 72, 4.4.1.4.2.3.

Based on observations during the facility walk-through life safety code inspection on 02/26-27/2014, with the facility VP, director of engineering, safety manager, and maintenance personnel, ity was noted that specific medical gas tanks (nitrous oxide) at the manifold location, were not secured per NFPA 99, 5.1.3.3.2(7). the nitrous oxide tanks were chained in a grouping with no chaines on two sides of the grouping, which could allow tank overturning.

Based on a review of records during the facility life safety survey conducted on siste on 02/26-27/2014, it was noted that the facility had no documentation indicating testing of the electrical receptacles in patient care areas on an annual (non-critical care patient areas) or semi-annual basis (critical care patient areas). Discussions with the director of engineering, safety manager, and VP administration, during this life safety code survey on the facility on 02/26-27/2014, verified that testing of the electircal outlets in patient care areas was not tested on a regualr basis for physical intergrity of receptacle, continuity ground for each receptacle, correct polarity of hot and netural connections at each receptacle, and retention force of the grouding blade of each receptacle.
