Based observation and staff interview the facility failed to provide patients with the Ohio Department of Health complaint hotline number. This has the potential to affect all patients receiving services from the facility. The facility's census was 129 at the time of the survey.
Findings include:
Review of the Patient Rights booklet provided to patients on admission revealed no notation of the complaint hotline number to the Ohio Department of Health. This was confirmed by Staff F on 02/19/16 at 2:40 PM.
