Building Construction Type and Height

Based on observation and interview, it was determined the facility failed to maintain building construction requirements in three locations, affecting one of three floors.

Findings include:

1. Observation on January 27, 2020, at 8:55 a.m., revealed structural steel lintels, located within the Sterilizer Room, lacked spray fireproofing.

Exit interview with the facilities manager on January 28, 2020, between 10:15 a.m. and 10:30 a.m., confirmed the building construction deficiency.

Vertical Openings - Enclosure

Based on observation and interview, it was determined the facility failed to construct and maintain vertical openings in two locations, affecting two of two floors.

Findings include:

1. Observation on January 27, 2020, between 12:45 p.m. and 2:00 p.m., revealed horizontal structural steel, located within the elevator enclosure at the first and second floors, was unprotected.

Exit interview with the facilities manager on January 28, 2020, between 10:15 a.m. and 10:30 a.m., confirmed the vertical opening deficiencies.

Hazardous Areas - Enclosure

Based on observation and interview, it was determined the facility failed to maintain one hazardous area enclosure, affecting one of three floors.

Findings include:

1. Observation on January 27, 2020, at 9:19 a.m., revealed the third floor Equipment Storage Room 308 door did not fully close nor latch.

Exit interview with the facilities manager on January 28, 2020, between 10:15 a.m. and 10:30 a.m., confirmed the hazardous area enclosure deficiency.

Sprinkler System - Maintenance and Testing

Based on observation and interview, it was determined the facility failed to maintain the automatic sprinkler system in one location, affecting on of two floors.

Findings include:

1. Observation on January 27, 2020, at 12:50 p.m., revealed a ceiling tile was missing within the second floor Data Room.

Exit interview with the facilities manager on January 28, 2020, between 10:15 a.m. and 10:30 a.m., confirmed the automatic sprinkler system deficiency.

Portable Fire Extinguishers

Based on observation and interview, it was determined the facility failed to maintain one portable fire extinguishing device, affecting one of two floors.

Findings include:

1. Observation on January 27, 2020, at 11:22 a.m., revealed the flush-mounted fire extinguisher bulb, located closest to the Accounting Services Office, lacked illumination.

Exit interview with the facilities manager on January 28, 2020, between 10:15 a.m. and 10:30 a.m., confirmed the portable fire extinguisher deficiency.
