Based on observation, record review, and interview, the facility did not identify laboratory (lab) personnel were eating, drinking, and storing perishable items in 1 of 1 room (break room) located in the laboratory area. A refrigerator in the break room that stored multiple blood and urine specimens was found to have 2 cartons of milk and protein shake on 3/29/16.

Findings included:

During a tour of the laboratory on 3/29/16 at 10:10 AM with Personnel #3, a "break room" was observed located in the the laboratory area. The following was observed in the break room:

1- Inside the room was a small table with 3 chairs. On top of the table were the following perishable items: half a bag-full of a 12 ounce "On the Border" chips, a bottle of used "Mae Ploy" sweet chili sauce (24 ounces), a half bottle of 16 ounce bottled water, 2 Styrofoam coffee cups with unsecured lids, and 1 personal travel cup.

2- There was a two-door refrigerator that stored multiple blood and urine specimens. Inside the refrigerator were 2 perishable items: one 8-ounce Highland 2% milk and one 11-ounce Premier protein shake.

3- Towards the back of the room was a filing cabinet and on top of the cabinet were as follows:
- A food tray inside a yellowish transparent biohazard plastic bag. The tray contained 13 single-serve cake-like desserts.
- An open box of multiple single-use coffee
- Multiple single-use catsup
- A box of Quaker granola bars
- Single-use Land O'Lakes butter
- Single-use coffee creamer
- A quarter bag-full of 24 ounce popcorn
- Multiple paper plates, napkins, plastic forks, spoons, and knives
- A large jar of Trailer Mix
- A large bag of Cranberry Pecan Granola

In an interview on 3/29/16 at 10:30 AM in the conference room, Personnel #3 was informed of the above findings. She confirmed the findings and stated the lab employees ate and drink in the lab break room.

"Laboratory Exposure Control Plan" revised 3/6/15 required "...Eating, drinking, smoking, applying cosmetics or lip balm, and handling of contact lenses are prohibited...Food and drinks are not stored in refrigerators...where blood or other...infectious materials are stored..."
