During the Revist Survey conducted on 10/19/2015, it was determined based on observations, interview, and review of Springbrook Behavioral Health's approved plan of correction, the hospital failed to implement it's plan of correction for ensuring removal of expired foods, and failed to ensure that the hospital had an adequate amount of water to meet emergency situations.
The findings are:
Observations in the hospital's dietary area on 10/19/15 at 11:25 a.m. revealed expired supplies in the hospital's wale in cooler: 2 containers of Cottage Cheese 4% with expirations dates of 09/10/15 and 10/01/2015; 1 Greek Vanilla Yogurt with an expiration date of 10/08/2015; 6 Yoplait Peach Yogurts with an expiration date of 09/06/2015; 4 Yoplait Strawberry Yogurts with an expiration date of 08/25/2015; 3 loaves Italian bread with an expiration dates of 09/28/2015, 09/28/2015, and 09/27/2015; and 1 half gallon of whole cultured buttermilk with an expiration date of 09/29/2015.
On 10/19/2015 at 11:38 a.m., during an interview with the dietary Manager, the audit tool as listed in the hospital's plan of correction for monitoring the walk in cooler, refrigerators, and freezer was requested, but the audit tools were never provided during the revisit survey. On 10/19/2015 at 11:40 a.m., the dietary Manager reported that everyone is responsible for monitoring the walk in cooler or walk in freezer.
On 10/19/2015 at 11:45 a.m., observations for the hospital's emergency water supply revealed 10 (5 gallon) jugs of water used in the water cooler was located on the floor by the water cooler in the medical records department. Observations showed 7 ( 5 gallon jugs) of water for the water cooler located on the floor of the business office. On 10/19/2015 at 11:47 a.m., the Facility Manager reported that the water in the water coolers was accessible any time by any staff, but that is the only stored supply of water that the hospital has.
