Based on observation and staff interview, the facility failed to ensure that their doors protecting corridor openings were maintained per Life Safety Code (LSC) 2000, 19.3.6.3. This deficiency has the potential to affect all residents, employees and visitors located within the smoke compartment.

Findings include:

1. Observation during facility tour on 08/12/15 at approximately 1040 hrs revealed room #75 door was propped open with a door-stop and would not resist the passage of smoke. This was verified on 08/12/15 at the time of discovery (during walk-through) by the Director of Maintenance. Exit interview with Director of Maintenance on 08/12/15 at approximately 1100 hrs verified knowledge of room #75 door being propped open with a door-stop and not resisting the passage of smoke.

Based on observation and staff interview, the facility failed to ensure that their hazardous areas were protected per Life Safety Code (LSC) 2000, 8.4.1, 19.3.5.4 and 19.3.2.1. This deficiency has the potential to affect all residents, employees and visitors located within the smoke compartment.

Findings include:

1. Observation during facility tour on 08/12/15 at approximately 1000 hrs revealed storage room door located within the kitchen was hitting frame, due to bad top hinge and would not self close/resist the passage of smoke. This was verified on 08/12/15 at the time of discovery (during walk-through) by the Director of Maintenance. Exit interview with Director of Maintenance on 08/12/15 at approximately 1100 hrs verified knowledge of storage door hitting frame and not self-closing/resisting the passage of smoke.
