Based on the life safety code inspection conducted on 11/15/12, it was determined the hospital was not maintained in a manner safe from fire. This had the potential to affect all those utilizing the hospital's facilities. The patient census at the time of the survey was 211.

Based on facility observation and staff interview and verification, the facility failed to maintain safety from fire. One of three offsite locations (a free standing medical center) was affected. Potentially any person utilizing the facility could be affected.
Findings included;
On 11/15/12 at 10:50 A.M. tour of the offsite location was conducted with Staff A, B and C. to determine compliance of findings noted during a survey completed on 10/04/12.
The offsite location of the facility provided urgent medical care, with basic radiology and some laboratory testing. Oxygen for patient care usage was provided by H and E sized cylinders stored outside the facility.
K130 was cited which addressed the facility failure to ensure that oxygen storage was in accordance with NFPA 99, Chapter 4, with regards to protection and storage of oxygen cylinders from excessive heat or freezing temperatures.
