Based on policy review, clinical record review and interview, it was determined the Facility failed to document baths for four (Patient #8 and #10-#12) of seven (Patient #7-#13) 4th floor patients. The failed practice did not ensure staff was aware of the patient's bathing needs and had the likelihood to affect any patient on the 4th floor. Findings follow:

A. Review of clinical records revealed the following:
1) Patient # 8 - Admitted on 03/31/16. No bath was documented for 9 of 11 days.
2) Patient #10 - Admitted on 04/04/16. No bath was documented for 4 of 7 days
3) Patient #11 - Admitted on 04/02/16. No bath was documented for 3 of 10 days.
4) Patient #12 - Admitted on 04/05/16. No bath was documented for 7 of 7 days.

B. During the time of record review 04/13/16 from 1300-1400, the Director of Medical/Surgical Services confirmed the baths were not documented.

C. During an interview on 04/14/16 at 1030, the Director of Critical Care Services confirmed the Facility did not have a policy regarding baths.
