Based on observation and interview, the facility failed to maintain a clean and sanitary environment by ensuring clean linen is free from contaminants. This failed practice is likely to lead to the transmission of diseases.
The findings are:
A. On 06/15/2021 at 10:30 am, observation of the Laundry Room area during facility tour revealed a soiled face mask and cigarette pack with 2 lighters on the shelf with clean linens.
B. On 06/15/2021 at 10:31 am, during interview with S2 (CEO), confirmed the Laundry Room area was soiled and instructed the housekeeper to clean the area.

Based on observation and interview, the facility failed to post Patient Rights (basic rule of conduct between patients and facility) in the Emergency Department area. This failed practice can cause neglect of patient autonomy (the right to self govern) and participation in care.
The findings are:
A. On 06/15/2021 at 8:28 am observation of the facilities Emergency Department during tour revealed the lack of signage "Patient Rights" posted in the area.
B. On 06/15/2021 at 8:29 am during interview with S (Staff) 2 (Chief Executive Officer) confirmed no signage of "Patient Rights" in the Emergency Department area stating, " I will take care of that, I'm not sure where it went."
