Based on interview and record review, it was confirmed that the hospital failed to ensure that 4(Staff B, E, F and H) of 8 sampled dietary personnel received orientation, education, and adequate specialized training appropriate to the tasks assigned specific to food handling and storage, infection control.
Findings:
Review of 8 sampled dietary personnel files revealed 4 of 8 employees (Staff B, E, F and H) did not have evidence that they received instructions or training on infection control, food handling preparation, serving and storage, cleaning and safe operation of equipment, and diet instructions.
During an interview on 2/11/2016 at 1:04 PM,with Director of Dietary who confirmed that Staff B, E, F and H did not have evidence in their individual education personnel files that they have received instructions or training on infection control food handling preparation, serving and storage, cleaning and safe operation of equipment, and diet instructions. Dietary Director stated that the weekend staff had not received the training and said she is currently developing a weekend training program.
During an interview on 2/11/2016 at 10:20 AM,with Staff K, Registered Dietitian (RD), stated that the Dietary department is contracted out with Food Management Group (FMG) based in Virginia. All the dietary employees work for FMG and not for Promise Hospital. RD stated that she is a full time RD employed by Promise Hospital of Florida at The Villages.
During an interview on 2/11/2016 at 11:31AM, with Staff K, states that most of the dietary staff works here as their second job, so they do not usually come for their scheduled in services.
Staff B was hired on 1/8/2014 as a dietary aide. Review of her education file provided by the Dietary Director on 2/11/2016 at 1:00 PM revealed an employee orientation checklist dated and signed by Staff B on 10/20/2014, 9 months after Staff B was hired. The checklist revealed competency on equipment use such as the dishwasher, slicer, mixer, cookie oven, cash register, steamer, set-up of pot sink, delivery of patients ' trays, set - up dessert, salads and hot food in café and set-up of dish machine area. There is no evidence that Staff B received training on food handling, food storage, personal hygiene and infection control.
during an interview on 2/11/2016 at 1:05 PM, Dietary Director confirmed that Staff B did not have evidence of such training on personnel education file.
Staff E, a dietary aide has a hire date of 6/11/2014. Review of the orientation checklist provided by the Dietary Director on 2/11/2016 at 3:00 PM revealed an incomplete checklist. Page 4 of 4 of the checklist was left blank regarding equipment use / competency. The checklist was not signed as completed by the employee. Dietary Director reviewed the employee file and confirmed the orientation checklist was incomplete and stated that the employee works during the weekend and that she is in the process of developing a weekend training program.
Review of employee files shows there is no evidence that Staff E received any training on food handling, food storage, personal hygiene and infection control.
Staff F was hired on 2/13/2015 as the weekend cook. Personnel file provided by the Dietary Director on 2/11/2016 at 3:00 PM revealed a signed copy of the employees ' job description signed on 2/13/2015. Page 3 of 3 of the job description under # 25 reads: Attends ALL Mandatory employee in-services as scheduled. Review of Staff F education file did not reveal evidence that Staff F received training on food handling, food storage, personal hygiene and infection control.
Staff H was hired on 12/18/2015 as a dietary aide and has been terminated. Review of the orientation checklist provided by the Dietary Director on 2/11/2016 at 1:00 PM did not reveal an employee orientation checklist. Dietary Director reviewed the employee file and confirmed the orientation checklist was not completed. Dietary Director on an interview on 2/11/2016 at 1:22 PM stated that Staff H was hired as dietary aide and to be trained as a part time cook, she did not show up for work in 2 occasions. Dietary Director provided a dietary training manual that she took home with her to study but had not returned the manual. Review of Staff H timesheet provided by the Director revealed she worked in the facility for a total of 6 days. There is no evidence that Staff H received any training on food handling, food storage, personal hygiene and infection control. No evidence on file for employee orientation checklist. Dietary Director stated that these checklist instructions should be provided to new personnel during orientation. Stated they cannot start to work until they have completed the checklist and training.
During an interview on 02/11/16 at 3:25 PM, Dietary Director, stated that staff E, F and H, did not have the completed checklist or training.
Review of the Dietary Policy and Procedure provided by the Dietary Director on 2/11/2016 at 3:35 PM on Intra-Departmental Inservice Training revealed an effective date of 7/1/2011 and Revision date of 5/20/2014.
Policy reads: In- service training is provided to Food Service Department employees on a monthly basis to improve understanding of job tasks and responsibilities.
Page 4 of 5 of the policy under item # 3 reads: Documentation of training and education is maintained in the individual employee training record, or the Management / Dietitian Continuing education record. For Food Management Group (FMG) employees, this information is also placed in the employee's personnel file.
