5.1.3.3.2 Design and Construction. Locations for central supply systems and the storage of medical gases shall meet the following requirements:
(1) Be constructed with access to move cylinders, equipment and so forth, in and out of the location.
(2) Be secured with lockable doors or gates or otherwise secured.
(3) If outdoors, be constructed with an enclosure (walls or fencing) constructed of non-combustible materials.
(4) If indoors, be constructed and use interior finishes of non-combustible or limited-combustible materials such that all walls, floors, ceilings and doors are of minimum 1 hour fire resistance rating.
(5) Be compliant with NFPA 70 NATIONAL ELECTRIC CODE, for ordinary locations, with electrical devices located at or above 5 feet above finished floor to avoid physical damage.
(6) Be heated by indirect means if heat is required.
(7) Be provided with racks, chains, or other fasteners to individually secure all cylinders, whether connected, unconnected, full, or empty, from falling.
(8) Be supplied with electrical power compliant with the requirements for an essential electrical system.
(9) Have racks, shelves, and supports, where provided, constructed of non-combustible or limited-combustible materials.
Based on observations and interviews the facility failed to ensure that medical gas cylinders are individually secured to prevent falling.
Observations during tour on 05/05/16 between 10:00 a.m. and 11:00 a.m. with Staff A (Director of Facilities Management & Security) and Staff B ( Assistant Director of Facilities Management & Security) revealed that inside the Medical Gas Manifold room there were several medical gas cylinders lined up around the exterior walls, that were secured together in groups or not secured at all. A group of 6 unsecured cylinders were in the middle of the room. The cylinders that were connected to the Gas Manifold System were only secured by the manifold connection.
The walls of the Gas Manifold Room have a vinyl trim baseboard installed around the the perimeter. This baseboard is a combustible product.
