Based on observation the facility failed to ensure 2 hour fire barrier protection seperating the doctor's clinic business occupancy and the health occupancy. Findings:
On 11/12/15 at 2:56 p.m., on inspection of the business occupancy doctor's office clinic on the 3rd floor it was observed there was no 2 hour fire barrier seperating it from the hospital.
At 2:56 p.m., the Director of Physical Services & Safety acknowledged the doctor's office clinic had no fire barrier seperation from the hosptial health occupancy.

Based on observation it was determined the faclity failed to ensure all doors protecting corridor openings were provided with a means suitable for keeping the door closed. Findings:
On 11/13/15 at 12:04 p.m., the housekeeping closet across from operating room #4 did not have self-closing hardware.
At 12:04 p.m., the Director of Physical Sevices & Safety acknowledged the door without self-closing hardware.

Based on observation it was determined the facility failed to ensure all smoke barriers were free of penetrations. Findings:
On 11/12/15 at 4:02 p.m., the 3rd floor smoke barrier near the double doors leading to the elevator was observed to have 2 penetrations.
At 4:02 p.m., the Director of Physical Services & Safety acknowledged the penetrations.
On 11/13/15 at 11:49 a.m., the 2nd floor smoke barrier next to housekeeping had 1 penetration. The IT closet on the 2nd floor had 2 penetrations.
At 11:49 a.m., the Director of Physical Services & Safety acknowledged the penetrations.

On 11/12/15 at 3:01 p.m., the air handeling unit room #3 was observed to have 2 penetrations.
The Director of Physical Plant & Safety acknowledged the 2 penetrations.
