Based on observations, the facility failed to maintain fire doors in operational condition. Fire doors help to contain hazardous conditions and the failure of these doors endangers all persons within the facility by allowing the passage of smoke, flames, noxious gases, etc. into adjoining compartments.

The findings include:

During the Specialty Care facility tour with the Director of Maintenance on 3/19/2014 beginning at approximately 10:00am, it was found that the fire doors next to room 419 were missing the latch which held the door closed. Fire doors must be self-closing and latch. The Fire doors are not in accordance with NFPA 101, 19 and 7.2.1
