MAINTENANCE OF PHYSICAL PLANT

Based on observation throughout the facility accompanied by staff, areas of the physical plant failed to be maintained for the safety of patients. The areas in need of repair include the patient kitchen area of the nursing station; floor and halls in surgery and the ceiling and tables in the laundry.

Findings were:
During the tour on 10/31/11 the the patient kitchen area of the nursing station was observed to have chipped paint on the cabinets where patient food is stored, these areas are not smooth and cleanable. In the surgery suite there were two areas on the hall corners where the tiles were missing withe the plaster exposed and these were not cleanable. Also in the operating room the tiles on the floor under and around the operating table were observed to be loose and discolored around the edges; and there were eight loose tiles pulling up from the floor. In the area of the in house laundry there were missing and badly discolored ceiling tiles, allowing exposed areas of the ceiling. Additionally in the clean side of the laundry the tables used for folding laundry were observed to be covered with contact paper that was chipped and peeling this was not smooth and cleanable. Interviews on 11/1/11 with the hospital administrator and nursing director confirmed these findings.

FACILITIES, SUPPLIES, EQUIPMENT MAINTENANCE

Based on observation during a tour of the hospital, all supplies, and equipment failed to be maintained for the safety of patients in the emergency department and supplies were expired.

Findings were:

Observation in the emergency department revealed that the beds in rooms 3 and 4 had covers that were ripped and torn areas that were not cleanable. In storage areas throughout the hospital the following supplies were observed to be expired - one Foley catheter kit 2010; 10 needles and three boxes of suture material 2009. Interviews on 11/1/11 with the hospital administrator and nursing director confirmed these findings.
