The findings include:
It was observed at approximately 10:00am through 3:00pm on November 25th, 2019 through December 2nd, 2019, that there were ceiling tile penetrations found in the following locations:
a) Inside of room M7319
b) In front of M7314
c) Main 7 east
d) Inside of electrical closet next to M6101
e) Replacement needed inside of M5318
f) Three (3) ceiling tiles M4311
g) In front of M1004
h) Two (2) inside of room GG011
i) In front of C4
Creating a potential hazard in an event of an emergency.

Based on observation, the facility had dust buildup on forty (14) out of 7000 sprinkler heads.
The findings include:
It was observed at approximately 10:00am through 3:00pm on November 25th, 2019 through December 2nd, 2019, the facility sprinkler heads in the following areas are dust laden:
a) Two (2) out of six inside of (ENT) waiting room 1st floor Gorman building
b) Two (2) out of 10 inside of emergency room nurses station C4205 and C22078
c) Room C6102 six (6) out of 10
d) C5214
e) C6114
f) C5200
e) C5304
Posing as a potential hazard in an event of an emergency.
