Based on observation and interview the facility failed to ensure door openings closed to resist the passage of smoke into an exit corridor. This could potentially expose patients, residents, staff, and visitors to smoke who used the activity area and corridor for egress. Findings:
Observation during the facility tour on 9/21/2011 at 10:13 am revealed the door to the orthopedic/casting room did not close and latch when tested. The door not properly closing would compromise individuals occupying two smoke compartments.
These observations were acknowledged by the Building Maintenance Director.

Based on observation and staff interview the facility failed to ensure emergency lights were functional to assist with facility evacuation in the event of a power outage. This could potentially affect patients, residents, staff, and visitors evacuating the facility in an emergency. Findings:
Observation during the facility tour on 9/21/2011 at 11:02 am revealed a battery-powered emergency light, by the main entry near the reception desk, failed to function when tested.
Further observation during the facility tour at 11:24 am revealed an emergency light in the boiler room by the medical air compressors that failed to operate when tested.
These observations were acknowledged by the Building Maintenance Director.
