Based on observations, record review and interviews, the CAH failed to maintain a clean and sanitary environment as evidenced by: 1) having grime and debris in patient rooms designated as cleaned and ready for patient admission and; 2) having grime and debris on the blood bank centrifuge located in the laboratory.
Findings:
1) Review of the hospital's policy/procedure titled, "Environmental Cleaning" revealed in part: Purpose - To maintain a clean environment for patients and minimize the risk of patient and healthcare personnel exposure to potentially infectious microorganisms.
Policy - The patient care environment throughout the facility will be maintained in a state of cleanliness that meets professional standards in order to protect patients and healthcare personnel from potentially infectious microorganisms.
Procedure - Other patient care areas and environmental surfaces that come in direct contact with patients will be cleaned with a facility-approved, EPA registered disinfectant. Check all furnishings and horizontal surfaces in the room including chairs, window sill, television, telephone, computer keypads, tables or desks.
Observations on 02/27/23 at 10:30 a.m. of the patient rooms designated as clean and ready for patient admission revealed: room a had an IV pole with old tape stuck to it and an over-bed table with a sticky substance on the top of it; room b had a used disposable thermometer cover on the window sill; room c had a cardiac/transport chair with grime, debris and hair on the lower aspects of the chair.
An interview with S1DON at this time confirmed that rooms a, b and c were designated as cleaned and ready for new admits.
2) Review of the lab's centrifuge operator's manual revealed in part: Cleaning and Decontamination of Spills - It is recommended that all interior surfaces of the centrifuge be cleaned periodically with a clean cloth dampened with a 10% bleach solution. The exterior surfaces can also be cleaned occasionally with the same solution.
Observations of the clinical laboratory on 02/27/2023 at 1:45 p.m. revealed grime and debris on the blood bank centrifuge.
In an interview at this time, S6Laboratory Manager acknowledged the grime and debris on the centrifuge.

Based on record review and interview, the CAH failed to ensure patients admitted to the swing beds were informed of their SNF patients' rights on admission by failing to include all the patient's rights provided to patients admitted to the swing beds.
Review of the patient rights supplied to the current swing bed patients in the CAH revealed the following SNF patient rights were not include in the information provided to the patients:
The right to retain and use personal possessions, including furnishings, and clothing, as space permits, unless to do so would infringe upon the rights or health and safety of other residents
The resident has the right to send and receive mail, and to receive letters, packages and other materials delivered to the facility for the resident through a means other than a postal service, including the right to: privacy of such communications consistent with this section; and access to stationery, postage, and writing implements at the resident's own expense.
In an interview on 02/28/2022 at 9:00 a.m., S2OpOfficer reviewed the rights provided to the patients and confirmed the right to retain and use personal possessions and the right to send and receive mail were not included in the patient's swing bed rights.
