Based on observation, document review, and interview, the facility failed to ensure Hospital Security, who assist and monitor patients in restraints, are trained in the use of First Aid, and certified in the use of Cardio-Pulmonary Resuscitation (CPR).
This failure places all patients at risk for potential harm.

Based on document review and interview, in 15 of 29 personnel files, the facility did not ensure that Hospital Security, who assist and monitor patients on restraints, are trained in the use of First Aid and certified in the use of Cardiopulmonary Resuscitation (CPR). (Staff D, E, F through R)
Findings include:
During interview with Staff A (Director of Hospital Security), and Staff S (Registered Nurse Assistant Administrator and Trainer) on 10/5/16 at approximately 12:55 PM, these staff members acknowledged that Hospital Security officers assist and monitor patients on restraints, but received no training in the use of First Aid, and were not certified in the use of Cardiopulmonary Resuscitation.
During interview with five (5) Hospital Security staff, on 10/5/16, staff confirmed that they assist the clinical staff during restraint application by manual hold, and have not received training in the use of First Aid and certified in the use of Cardiopulmonary Resuscitation (CPR).
Specifically, during multiple interviews with five (5) hospital security officers on 10/5/16, the following responses were documented Staff C interviewed at 2:45 PM, stated that she assist holding patients during restraint application under the direction of the clinical staff, did not receive training on First Aid, and certified in CPR; Staff B interviewed at 3:49 PM, stated that security hold patient during restraint application under directives from a nurse or doctor, did not receive training in the use of first aid, and certified in CPR. Similar information were obtained and validated during interviews with Staff D, E, and F.
Review of the facility's policies and procedures on "Restraints" last revised on 2/16, and titled "Searching, Restraining and Monitoring of Psychiatric Patients" with effective date 12/2015, identified that Hospital Security officers will visually monitor patients, and assist medical and nursing staff by holding the patient down during the application of restraint.
