Based on observations made during tour of the facility on 2/1/10, it was determined the facility failed to ensure the exit access corridors were clear and unobstructed at all times, to full instant use in case of fire or other emergency. This in the event of fire or any reason to evacuate from a portion of the facility to another section or outside for an area of refuge may be delayed or denied as a result of exit corridor obstruction.

The findings include:

Between 4:15 p.m. and 4:45 p.m. on 2/1/10, the wallaroo cabinets mounted to the corridor walls in patient corridors were observed to be held open. The auto close devises were negated by placing objects that weigh enough to hold the doors open. This was observed at patient room 206, by a food tray, patient room 204, by an accucheck machine, and a food tray by patient room 304. This creates a protrusion into the clear width of the corridor and creates an obstruction. Protrusions into a corridor are limited to 3 1/2 inches. The wallaroo doors are over one foot when open.

Based on a review of the facility records and interview with the staff on 2/2/10, it was determined the facility failed to continually operate under an approved Comprehensive Emergency Management Plan. An unapproved plan may be inaccurate and cause confusion and/or panic from a lack of knowledge for staff and raise potential for negative outcomes to the patients and staff, and other building occupants.

The findings include:

The Collier Campus Disaster Plan expired on 10/7/09. The plan was not resubmitted for approval until 10/8/09, and the approval from Collier County Emergency Management was not obtained until 11/18/09. This left the facility operating without an approved plan for 42 days.
