Based on review of the medical record, facility policies and staff interview the emergency department staff restrained a patient without any physician order.
Findings were:
On 12/23/09 at 1920, patient #1 left the hospital barefoot and in only a hospital gown. (On 12/23/09 the weather in Lubbock, TX was stormy all day with a mixture of rain and snow precipitation) Two nurses walking to their cars recognized the patient. They stopped her and escorted her back to the building with a security guard. Once in the building she was placed in a room alone with the door closed and blinds drawn; her behavior was documented as agitated, impulsive, inappropriate and uncooperative.
After being brought back to the ED room, this patient was placed in handcuffs by the security guard who is a DPS officer. There was not a physician order for these restraints.
Interview with the Risk Manager and Accreditation Coordinator confirmed the above findings.
