Based on observation, interview and record review the facility failed to maintain a complete inventory of all facility equipment used in the care of patients and failed to provide ongoing preventative maintenance records according to manufacturer's guidelines for 17 of 24 sampled pieces of medical equipment. These 17 items used for patient care were found in six (6) different departments throughout the hospital.
Findings include:
Interview with the Chief Nursing Officer, (CNO), on 5/27/2021 at 11:15am revealed that a complete inventory list was not maintained by facility staff. The facility currently contracts with a vendor whose technician was currently responsible for maintaining the safety log of all facility medical equipment. Facility did not have any department specific equipment inventory to track preventative maintenance for patient care equipment.
A. Cardiac Rehabilitation - NuStep Exercise Bike #4276
This piece of equipment was not in the current number logging system nor was an inventory sheet available for this piece of equipment. Interview with CNO on 5/27/21 at 11:15am revealed this piece of equipment had been serviced previously by another vendor and the exercise bike did not get put on the current contracted service list.
B. Eight (8) pieces of patient care equipment available for current use had no information available regarding last safety inspection in the master log book, including:
39 - Centrifuge (device used in laboratories to separate blood and fluid components) - located in facility lab department.
249 - Centrifuge- located in facility lab department.
251 - Monitor screen - located in cardiac rehab department.
170 - Microscope - located in the specialty clinic for patient examination.
6 - EKG (electrocardiogram) - used for measuring cardiac rhythm - located in specialty clinic.
74 - Procedural light for patient exams - located in specialty clinic.
77 - Procedural light for patient exams - located in specialty clinic.

C. Eight (8) additional pieces of patient care equipment available for immediate use had data sheets in the master inspection log book with no date of service for last safety inspection:
8 - Blanket warmer - surgery department.
463 - Medical grade refrigerator - located in surgery department.
128 - Non Invasive Blood Pressure Monitor - cardiac rehabilitation.
419 - Vital Sign Monitor - (for measurement of blood pressure, heart rate and oxygen saturation) - specialty clinic.
93 - Defibrillator - (therapeutic shock delivery for heart rhythm) - emergency room.
228 - Physiological monitor - (measures blood pressure, heart rhythm, oxygenation with alarms) - emergency room.
62 - Electric stimulator (therapy/muscle treatment) - therapy department.
474 - Electric stimulator (therapy/muscle treatment) - therapy department.
