Based on observation and staff interview, the facility failed to separate a hazardous area with a smoke resistive door. This condition would allow smoke to migrate into the exit corridors.

Findings are:
Observation on 5/27/21, at 11:30 am revealed Operating Room 2 was used as a storage room for extra beds and combustible medical supplies. A strike was not installed in the door handle, and the door failed to latch when self-closed.

In an interview on 5/27/21, at 11:30 am, Maintenance A acknowledged the door did not positively latch.
