Based on interview and record review the facility failed to have an individualized comprehensive treatment plan in that, 1 of 1 patients (Patient #3) comprehensive treatment plan did not include sexually acting out behaviors.

Findings included:

Patient #3 engaged in sexual contact with Patient #2 on the night of 05/15/21 in room 28. Multidisciplinary treatment plan update dated 5/17/21 at 1030. Did not address patient's sexual acting out behavior of 5/15/21.

During an interview with Personnel #1 on 06/14/21 at 1336 Personnel #1 verified the updated treatment plan did not address the sexually acting out behavior.

During an interview with Personnel #3 on 06/14/21 at 1359 Personnel #3 verified the updated treatment plan did not address the sexually acting out behavior.
