PATIENT RIGHTS: CARE IN SAFE SETTING

Based on Interview, Recording Review and Observation the hospital failed to ensure that patient 2 of 25 patients' received (Patient #9 and Patient #25) received care in a safe setting.

Findings Included:

During the interview process personnel #6 indicated that the hospital has failed to follow their own policy. If a patient is considered to be high risk of falling an assessments on each shift should be completed as the hospital policy states.

During the Records Review it was discovered that only two assessments reassessments had been completed based on the patient high risk for falls scores. The assessments were completed on the 14th and the 18th.

During the patient observation 2 of the patients observed were observed without non-skid socks or shoes on their feet. One of thos patients feet were extremely dirty.


Policy

The hospital Patient Rights Policy dated 06/2016 reflected, "It is the Policy of the hospital to ensure the patient's rights are protected and maintained. All patients are informed of their rights verbally and in writing upon admission. Any limitation of the patient's rights shall be made in accordance with applicable statues and rules and only if the exercise of those rights would constitute harm to the patient or others."

The hospital Assignment of Nursing Staff dated 10/2013 reflected, "To assure quality nursing and a safe patient environment, nursing personnel staffing and assignments are based on at least ...Patient care assignments is commensurate with the qualification of each nursing staff member and the identified nursing need of the patient according to acuity ..."
