Based on observation and interview the hospital failed to follow it's infection prevention and control program, employing methods for preventing and controlling the transmission of infections within the hospital in that 4 ( #1,#5, #6 and #7) Personnel failed to wear a face mask while on duty in the hospital.
Findings included:
On 03/08/21 at 1030 while walking in the hospital hallway Personnel #5 was seen at the nursing station not wearing a mask.
On 03/08/21 at 1035 while waiting in the hallway to meet with hospital administration Personnel #6 and Personnel #7 were seen walking in the hallway not wearing a mask.
On 03/08/21 at 1046 during the opening conference Personnel #1 did not wear a face mask while within 6 feet of the surveyor in his office.
During an interview on 03/08/21 at 1055 in the Conference Room Personnel #2 stated the hospital infection control policies were bases on the Centers for Disease Control (CDC guidelines. Personnel #2 acknowledged that Personnel #1 was not wearing a mask during the opening conference.
During an interview on 03/08/21 at 1110 in the conference room Personnel #3 acknowledged that Personnel #6 was not wearing a mask when he entered the conference during this survey.
The undated policy titled Covid-19 Personal Staff Infection Control reflected..." Wear a mask in all patient-care areas, hallways, and public areas..."
