Based on records reviewed and interviews, the Hospital failed to ensure that the Nursing Department policy and procedure titled Medication Reconciliation was reviewed and updated on 6/8/16 according to policy and procedure.
Findings include
The Nursing Department policy and procedure titled Medication Reconciliation indicated the last review date was 11/1/12 and was scheduled for review on 6/8/16, one year before the survey which occurred in June of 2017.
