Based on observation, it was determined that the facility failed to provide exit access at least 48" wide.
Findings include:
1. On 10/11/12 at 12:15 PM in the presence of Staff #39, it was observed that the rear exit to the Peds ED was restricted to 30" from the required 48", by 6 carts.
