The inspector observed, while accompanied by the CEO, CNO, and the Quality Director, during the hours of the inspection from 02:30 PM to 06:30 PM on 08/08/2019that there were the following issues. They were the following issues:
A) Some of the smoke barrier cross corridor doors were not fully closed to prevent the smoke migrate to other smoke compartments.

The inspector observed, while accompanied by the CEO, CNO, and the Quality Director, during the hours of the inspection from 02:30 PM to 06:30 PM on 08/08/2019that there were the following issues. They were the following issues.
(A) There was a missing the battery powered operated light fixture at the essential automatic transfer switch location and generator set location.
(B) The electrical panel and breaker number were not identified on the main FACP.

The inspector observed, while accompanied by the CEO, CNO, and the Quality Director during the hours of the inspection from 02:30 PM to 06:30 PM on 08/08/2019that there were the following issues. They were the following issues.
A) The combustible material plywood and wood studs was utilized for the medical gas storage room wall. Please utilize none-combustible material for medical gas storage room enclosure.
B) Some medical gas cylinders were not secure in place or individually chained.
