Facility failed to maintain auto dialer for fire alarm system in accordance with the 2000 edition of the Life Safety Code, NFPA ( Natitional Fire Protection Association) 101. Findings included the following:

Surveyor observed between 10 and 11am on 1/29/14 that fire alarm system auto dialer for hospital was in trouble during test of system.

Facility failed to maintain sprinkler coverage in General Storage Room in accordance with the Standard for Installation of Fire Sprinkler Systems. Findings included the following:

Surveyor observed between 10 and 11am on 1/29/14 that combustible cardboard boxes were stored within 18 inches of fire sprinklers in middle shelve of Room I-143 which does not meet NFPA 13.

Facility failed to maintain Type I EES in accordance with the Standard for Health Care, NPFA 99. Findings included the following:

Surveyor observed between 10 and 11am on 1/29/14 that in switch gear room, no battery powered lighting unit was installed as required.

Facility failed to maintain requireed records of receptacle testing in critical care areas in accordance with NFPA 99. Findings included the following:

Surveyor observed between 10 and 11 am on 1/29/14 that facility records did not include semi-annual grounding system tests in critical care areas. Receptacles shall be tested in Patient Care areas as follows:

Physical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shall be not less than 4 oz. (test)

Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.
