Based on observation, interview and record review, the hospital failed to ensure staff completed a daily crash bag checklist for one patient care unit of four patient care units observed.
This failed practice placed all patients at risk for their health and safety. The hospital census was 83.
Findings included:
Even though requested, the hospital failed to provide a crash bag policy.
Review of the North Kansas City (NKC) adult behavioral health unit daily crash bag checklist on 01/30/23, showed the following:
- No check on four out of 30 days for 09/2022;
- No check on 11 out of 31 days for 10/2022;
- No check on five out of 30 days for 11/2022;
- No check on 13 out of 31 days for 12/2022; and
- No check on 11 out of 25 days for 01/2023.
During an interview on 02/01/23 at 11:00 AM, Staff A, Director of Nursing (DON), stated that the house supervisor was responsible for checking the crash bag every night. Her expectation was that the checklist be completed daily.
