Based on findings from observations and interview, the access route leading from the room designated as day surgery to the exit stairwell located at the northwest corner of the hospital was not in compliance with NFPA 101, Chapter 9, due to a reduction in exit corridor width along the exit travel route.
Findings include:
--Per observation on 11/14/11 at 11:30 am, the travel route to access the northwest exit stairwell of the second floor west wing of the hospital from the room designated as day surgery requires travel through a corridor 8 ft in width to another corridor less than 8 ft. in width.
--Per interview with the Director of Facilities on 11/15/11 at 3:30 pm, a portion of the 8 ft. corridor originally used to access the stairwell is now part of space leased to a private physician. (This leased space obstructs access to the exit stairwell.)

Based on findings from observations and interview, the leased space on the second floor of the west wing of the Hospital was not separated from the Hospital by 2-hour fire rated construction as required by 2000 NFPA 101, Chapter 19.1.2.
Findings include:
--Per observations on 11/14/11 at 11:30 am, the double doors and wall above the double doors on the second floor of the west wing of the Hospital separating leased space from the Hospital were not two hour fire rated. The Director of Facilities who was present confirmed these findings.
--Per interview with the Director of Facilities on 11/15/11 at 3:30 pm, the separation between the leased space and hospital was largely one hour as opposed to two hour fire rated.
