Based upon observations made in the presence of the plant manager on 6/9/10, it was determined that the facility did not provide a two-hour fire resistance rating at the horizontal exit connecting the hospital lobby to the clinic area.
Findings include:
The 2 hour fire wall between the hospital lobby and the clinic area had a 4 ' ' hole in it above the lay in ceiling that would not resist the passage of fire or smoke.

Based upon observations made in the presence of the plant manager on 6/8/10, it was determined that the facility did not maintain hazardous areas to be fire and smoke separated from other sections of the facility.
Findings include:
1. The fire rated door to the ER Soiled Linen room was obstructed form self closing by a modified closing device.
2. The Labor & Delivery Soiled Linen room fire rated door lacks a self closing device.

Based upon observations made in the presence of the plant manager on 6/8/10, it was determined that the facility did not maintain the fire sprinkler system in accordance with NFPA 13 and CMS.
Findings include:
1. The fire sprinkler system lacks a current certification by a company that is licensed and certified in the state of Utah.

Based upon observations made in the presence of the plant manager on 6/8/10, it was determined that the facility did not maintain flame retardant treatment on combustible draperies, curtains and other loosely hanging fabrics in accordance with NFPA 101 10.3.1.
Findings include:
The three LDR rooms have fabric window valances that do not have labels stating the fabric is flame resistant.

Based upon observations made in the presence of the plant manager on 6/8/10, it was determined that the facility did not maintain medical gas storage in accordance with NFPA 99.
Findings include:
The Oxygen Manifold Supply room contained 9 unsecured " E " tanks.

Based upon observations made in the presence of the plant manager on 6/8/10, it was determined that the facility did not maintain, inspect and exercise the facilities emergency generator set in accordance with NFPA 99.
Findings include:
The facility emergency generator set does not annunciate to a constantly staffed location.
