Based on interview and record review, the facility failed to provide a patient, in an intermediate swing bed, with a 30-day notice of transfer/discharge when transferred to another acute care hospital for 1 (#1) of 2 sampled patients. Findings include:

During an interview on 10/03/23 at 8:10 a.m., NF1 stated patient #1 had an extensive history of mental health issues. NF1 stated when patient #1 was transferred to another facility for treatment in June 2023, he was not provided a 30-day notice of transfer/discharge. She stated patient #1 was unaware of his right to a fair hearing.

During an interview on 10/03/23 at 9:09 a.m., staff member B stated when a patient was admitted to an intermediate swing bed, they were given a notice of patient rights. She stated a 30-day notice of transfer/discharge was part of the patient rights information.

Review of patient #1's electronic medical record showed an inter-facility transfer authorization to [Facility Name] on 06/12/23, but failed to show a 30-day notice of transfer/discharge was provided to patient #1.

During an interview on 10/03/23 at 9:29 a.m., staff member B stated the facility did not have record of a 30-day notice of transfer/discharge for patient #1. Staff member B stated a 30-day notice of discharge should have been issued to patient #1 when he went to [Facility Name] for further treatment.
