Based on observation and staff interview, it was determined that the facility failed to provide a safe setting by ensuring that all equipment used to monitor vital signs in the units are kept secured when not in use.

Findings include:

On 1/26/23, during a tour of the Adult Treatment Unit, Dual Diagnosis Unit, and Older Adult Unit, Dinamap equipment (machine used to monitor vital signs) were observed stored outside the nurses' station. Presence of facility staff members were observed around the nurse's station, however, there was a concern that the equipment could be used as a weapon if left unsecured.

Upon interview with Staff #1 at 12:10 PM, it was confirmed that the Dinamap equipment should be stored behind the nurses station when not in use.
