Based on record review and interview the facility failed to specific treatment modalities utilized for 1 of 1 (Patient #1) patients in that the faciity did not document the medications used in treatment of the patient.
Findings included:
Patient #1 was admitted on 10/06/22 for suicidal ideation. The Master Treatment Plan does not reflect the patient was prescribed Zoloft 75mg, and Abilify 5mg daily
During an interview with Personnel # 1 on 11/14/22 at 1437 in the Conference Room, Personnel #1 verified the above findings.
