Based on observations and interview the hospital failed to ensure drugs that were outdated were not available for use as evidenced by expired drugs discovered in the CT scan room of the hospital in a kit labeled "Reaction Kit for CT Contrast".
Findings:
Observations, on 07/01/19 at 9:10 AM, of the CT scan room revealed a kit labeled "Reaction Kit for CT Contrast".
Located inside the reaction kit was: a. 2 vials Solu-Medrol 125 milligrams (mgs) per vial with expiration date of 12/2018 for both vials; b. 2 packets of Ranitidine Syrup 10mL (milliliters) 150mg/10mL with expiration date of 11/2018 for both; c. 1 vial of Diphenhydramine 50mg per mL (1mL vial) with an expiration date of 08/18.
Interviews, on 07/01/19 at 9:15 AM, with Staff #A and Staff #D confirmed the drugs were expired and should have been removed so they could not be used on patients.

Based on record review and interview the critical care hospital failed to evaluate services affecting patients' health and safety by not evaluating patients after antibiotics were administered and by not tracking causative organisms of infections.
Findings:
A review of the infection control logs for 01/2019 through 06/2019 revealed the critical access hospital failed to document causative organisms. Continued review revealed patients were not evaluated for signs symptoms of infections, after the last antibiotics were administered
A review of the Infection Control Meeting Minutes revealed the critical access hospital did not conduct meetings for the second quarter (04/2019 through 06/2019).
In an interview on 07/01/19, Staff #C stated she was the infection control officer. Also, Staff #C stated, when cultures are done, the lab does not send reports to the hospital. Staff #C confirmed that the hospital was not tracking organisms and evaluating patients after antibiotics were administered.

Based on record review and interview the critical access hospital failed to ensure the hospital had a medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident by not employing or contracting with a medically social consultant.
Findings:
A review of the policies and procedures for the critical access hospital revealed "social services are directed by a licensed medical social consultant and duties carried out by the social service designees".
On 07/01/19 at 11:10 AM Staff #A stated the hospital has not had a contract a licensed medical social consultant since 2011. Additionally, Staff #A said the hospital does not have an employee who perform those duties.
