Based on record review and staff interview, the facility failed to assure fire drills were scheduled and occurred at random times and in unexpected conditions. This practice can effect proper staff reactions to an actual emergency evacuation, affecting 5 of 5 smokes zones. This facility has a capacity of 38 beds and a census of 29 at the time of the survey.

Findings Include:

During the survey on 3/15/2011 between 12:00am and 3:00pm the folling was observed:

The facility failed to have random and unexpected fire drills on the 3rd shift (10:00pm - 6:00am) scheduling 3 of 4 quarters between 4:00am - 4:30am, creating a pattern for shift employees.

Staff member "A" acknowledged the pattern development and stated that they would help monitor the times as to not create any type of time patterns for all the shifts involved.

NFPA Standard: Requires drills be conducted at least quartely on each shift under varied conditions to simulate the unusual conditions occuring in case of fire. The fire alarm shall be transmitted during drills although a coded announcement may be used between 9:00pm to 6:00am. 2000 NFPA 101, 19.7.1.2
