Based on observation and interview combustible storage was stored outside of a hazardous area. This deficient practice could allow a fire to spread affecting approximately 12 occupants.

Findings are.

Observation and interview on 06-22-21 at 11:45 AM revealed combustible materials stored in the corridor adjacent to the Storage room..
During and interview on 06-22-21 at 12:15 PM with Maintenance Personnel and Administrator 1 the findings were verified.

Corridor - Doors

Based on observation and interview the facility failed to maintain the closing operation on the fire separation doors located in the west corridor adjacent to the business office. This deficient practice has the potential to allow fire and smoke to extend from the patient area into the corridor serving the front entrance, and business office area affecting approximately 6 occupants.

Findings are:

Observation and interview on 06-22-21 at 11:35 AM revealed that the fire separation doors separating the patient area of the facility and the business office and front lobby area failed to close and latch properly when released from the open position.

Interview with Maintenance personnel and Administrator 1 on 06-22-21 at 12:15 PM verified the findings.

Subdivision of Building Spaces - Smoke Barrie

Based on observation and interview the fire separation doors located in the corridor adjacent to the dining room, and the 100 patient corridor did not maintain a smoke tight seal when closed. This deficient practice would allow smoke to extend from these areas into the adjoining areas of the hospital affecting approximately 7 occupants.

Findings are:

Observation and interview on 06-22-21 at 10:45 AM, and 11:30 AM revealed a gap between the fire separation doors when in the closed position.

During an interview on 06-22-21 at 12:15 PM, Maintenance Personnel, and Administrator 1 verified the findings.
