Based on observation, 23 doors on the first floor of the Kilgore building had locks installed that are not operable from the interior of the room.

Findings include:

During the Life Safety survey conducted on January 17, 2012, while touring the interior of the facility between 9:30a.m. and 4:30p.m., former patient rooms on the first floor that were temporarily converted to other uses, had a surface mounted lock installed on the corridor side which was not operable from the interior of the room. All of these locks were removed during the survey.

Based on observation, a fire sprinkler head was obstructed with a piece of board on top of a sterilizer machine.

Findings include:

During the Life Safety survey conducted on January 17, 2012, while touring the interior of the facility between 9:30a.m. and 4:30p.m., while in the 2nd floor sterile equipment processing area (SPD) a section of what appeared to be wallboard about 2' X 3' was observed on top of one of the two large steam sterilizers in the room extending to the ceiling which obstructed a fire sprinkler head just a few inches away. It appeared the board was installed to prevent a steam leak from impinging on the sprinkler head and potentially fusing it. The machine was shut down and further use ceased pending repair of the steam line leak.

Based on observation, high pressure oxygen cylinders were observed at three locations unsecured to prevent accidental damage.

Findings include:

During the Life Safety survey conducted on January 17, 2012, while touring the interior of the facility between the hours of 9:30a.m. and 4:30 p.m., "E" size oxygen cylinders were observed in the O.R. corridor on two transport gurneys lying at the foot of the mattress without any type of restraint. Additionally single cylinders were observed in the Fast Track recovery area and the O.R. recovery area without proper restraint.
