Sprinkler System - Installation

Based on observation and staff interview the facility failed to provide a sprinkler system installation in accordance with NFPA 13 and NFPA 101, Section 19.3.5.1, resulting in the potential to impact all occupants of the facility in the event that the sprinkler system is damaged due to hangers being subjected to loads they were not designed for. Findings include:

A) On January 23, 2018 at approximately 1:37 P.M., a sprinkler system support was observed above the acoustic ceiling in the corridor outside smoke barrier door #8, which had a bundle of "Cat 5 communication cables", attached to the support by a plastic zip tie.

B) The finding described in "A" above was confirmed with staff "K" at the time of discovery. Staff "K" observed the identified condition and stated he had witnessed it.

Subdivision of Building Spaces - Smoke Barrie

Based on observation and staff interview, it was determined the facility failed to provide smoke barrier wall construction in accordance with NFPA 101, Section 19.3.7.3, this deficient practice has the potential to affect all occupants of the two adjoining smoke compartments in the event of smoke not being properly contained away from the unaffected compartment. Findings include:

A) On January 23, 2018 at approximately 1:50 p.m., an unprotected penetration was observed above the smoke barrier door between the hospital lobby and the emergency department. An approximately one inch hole was observed to be drilled through the wall with a bundle of "Cat 5 communication cables" passing through the hole. The penetration was not sealed against the passage of smoke or fire.

B) The finding described in "A" above was confirmed with staff "K" at the time of discovery. Staff "K" observed the identified condition and stated he had witnessed it.

Utilities - Gas and Electric

Based on observation and staff interview, it was determined the facility failed to provide an electrical installation in accordance with NFPA 70 (The National Electrical Code) and NFPA 101, Section 19.5.1.1, this deficient practice has the potential to affect the occupants of the emergency department by not protecting wiring from mechanical damage or contact that may serve as an ignition point. Findings include:

A) On January 23, 2018 at approximately 1:51 p.m., an open/uncovered electrical junction box was observed above the acoustic ceiling in the emergency department outside room "1".

B) The finding described in "A" above was confirmed with staff "K" at the time of discovery. Staff "K" observed the identified condition and stated he had witnessed it.
