IMPLEMENTATION OF A DISCHARGE PLAN

Based on record and interview the hospital failed to arrange for the initial implementation of the patient's discharge plan for each patient, in that 1 of 1(Patient #1) patient's discharge plan was not implemented prior to the patient being discharged from the hospital.

Findings Included:

Patient #1 was admitted on 12/20/16 for an Acute Stroke. On 12/22/16 Physical Therapy recommended discharge to an inpatient rehabilitation facility. Patient #1 was discharged on 12/25/16 with a list of places to buy a wheelchair. Discharge planning did not evidence any attempts to place patient in an inpatient rehabilitation facility, a skilled nursing facility, or an attempt to set up home health care.

During on interview on 05/16/17 at 1645 Personnel #4 stated the case manager responsible for the discharge planning on Patient #1 believed Patient #1 was on an indigent care program. Personnel #4 stated the hospital maintains a list of home health agencies that will accept indigent patients. Personnel #4 agreed there was not an implementation of the patients discharge plan.

The policy titled Discharge Planning dated 07/19/16 reflected..."The hospital shall arrange for the initial implementation of the discharge plan. This includes arranging for necessary post-hospital services and care..."
