Based on record review and interview the Hospital failed to complete a discharge plan that included recommendations for follow-up services concerning a safe discharge for 1 of 1 patient (Patient #3) who had tested positive for marijuana. Patient #3 was discharged to the same facililty that he was admitted from with a positive drug screen.
Findings Included:
The 03/23/15 Emergency Patient Record reflected, "Patient #3 was triaged by the police department to the hospital...was causing harm to himself by punching walls and attacking objects, people, and running up and down the street...had severe agitation, was unpredictable and had anxiety behaviors."
On 03/23/15 at 2100 the emergency patient record reflected, "Patient #3 had a urine drug screen...results of the drug screen were positive for "THC (marijuana)."
On 03/24/15 at 1135 Patient #3 was assessed by a physician and was determined to have tested positive for marjuana and "...seems severely MR and is unable to contribute anything meaningful to the interview..."
On 03/24/15 at 1201 Patient #3 was discharged with a referral for psychiatric services and hospital follow-up. There was no indication in the medical record that a referral was made to an agency to determine if Patient #3 was discharged to a safe environment.
During an interview on 7/09/15 at 1620 Personnel #8 stated that Patient #3 was not capable of obtaining marijuana on his own accord and knew how to smoke it. Personnel #8 said that he did not know why this was not reported to a protective agency. Personnel #8 was not sure why the discharge personnel or social services did not further investigate the home environment of Patient #3 for a safe living arrangement after testing positive for marijuana at admission.
